What usually causes it
A block failed its checks when read. Storage or I/O path problems are the common source.
What to check first
Run BACKUP VALIDATE CHECK LOGICAL DATABASE in RMAN to find every bad block, then RECOVER DATAFILE n BLOCK m from a good backup. Map file and block to a segment through DBA_EXTENTS.
