What usually causes it
The Fast Recovery Area reached DB_RECOVERY_FILE_DEST_SIZE. Usually reported with ORA-19804.
What to check first
Delete obsolete backups and backed-up archived logs through RMAN, or raise the size. Check for guaranteed restore points holding flashback logs.
