What usually causes it
An index and its table disagree, a logical corruption.
What to check first
Identify the index from obj# in DBA_OBJECTS, confirm with ANALYZE TABLE ... VALIDATE STRUCTURE CASCADE, then rebuild it ONLINE (which reads the table, not the damaged index) or drop and recreate it.
