ORA-01000: maximum open cursors exceeded

What usually causes it

A session hit OPEN_CURSORS, usually because the application doesn't close statements or result sets.

What to check first

Count open cursors per session in V$OPEN_CURSOR to find the offender. Raising OPEN_CURSORS (dynamic) only helps if the count is legitimately high.
